Patient Care Navigator Job Summary:

Patient Care Navigator provides coordinated care to patients by developing, monitoring, and evaluating interdisciplinary care.

Patient Care Navigator also performs routine administrative and clinical tasks to keep the office of physicians and other health practitioners running smoothly.

The duties of the Patient Care Navigator may vary by location within the company.

Patient Care Navigator Position Responsibilities:

Responsibilities of the Patient Care Navigator includes (but is not limited to):


• Answers phones and schedules appointments

• Prepares correspondence

• Processes mail distributed to them

• Maintains supplies and equipment

• Performs computer skills in patient data entry and accessing online imaging

• Scans documents into EMR

• Arranges for outside referrals and authorizations

• Schedules meetings with patients and their families

• Calls in prescriptions to the pharmacy

• Uses communication skills with appropriate medical terminology

• Follows appropriate legal and ethical professional conduct

• Assists physician with exams as necessary

• Performs accurate, legal, and ethical documentation at all times

• Maintains patient care database by entering new information as it becomes available; verifying    findings and reports; backing up data

• Performs other position-related duties as assigned.


• Follows facility and OSHA safety rules and procedures

• Proper disposal of medical waste

• Follows facility and GHR Unusual Occurrence Protocol

• Respects cultural and religious practices of patients

• Upholds HIPAA regulations

• Punctual and dependable

Patient Care Navigator Qualifications:


High school or commercial school graduate.

Two years of college preferred.

Work Experience:

Two years experience as a medical assistant and/or medical receptionist with preferred experience in clinical office and physician practice operations.


Pay and benefits:

The rate of pay is $16.00 – 18.00 hr. to start based on experience.   Our pay period is twice monthly.    Benefits offered include health insurance, paid holidays, vacation time and sick days as well as a 401K savings plan.


We are currently hiring for our clinic located in Clairemont, San Diego, CA 92111


Monday through Friday 7:30 am to 4:00 pm   (may occasionally start earlier and have a need to stay later)
